What is HomMed?

HomMed Health Monitoring System: Imagine the sense of security and the peace of mind you would have knowing that your health or the health of your loved one is being monitored by a registered nurse on a daily basis. Piedmont Home Care (PHC) can now provide you or your loved one with this sense of security using state-of-the-art telehealth technology from HomMed. HomMed is the leader in the rapidly-growing home patient telemonitoring industry.

How It Works

The HomMed Health Monitoring System is a simple-to-use health monitor that collects patient’s clinical vital signs every day. Using voice prompts and visual cues the monitor will guide the patient through the vital signs process measuring weight, temperature, blood pressure, blood oxygen saturation, and pulse. To get a more comprehensive input on patient’s health, the monitor will also ask up to ten health related questions. HomMed Monitor allows for multiple peripheral attachments, such as glucose meter, spirometer, PT/INR System, ECG unit, etc. All of the above data is immediately transmitted to our agency for a clinical review. If the data collected indicates a need for intervention or if the user fails to collect their vital signs when they are prompted to, appropriate action is taken and members of the care team may be contacted for intervention.

Benefits

HomMed Health Monitoring System:

  • Provides sense of security for the patient and patient’s family knowing that somebody is watching over their health every day.
  • Provides care based on patient needs, not only as nurse’s schedule allows.
  • Lets the doctor adjust medications or diet based on trend over time data.
  • Catches irregularities early, before they become problems: reduces emergent care episodes keeping the patient out of the ER and in the physician office.
  • Acts as an educational tool to reinforce medication use and diet restrictions.

The HomMed System is beneficial for almost anybody, especially helpful to those who have heart disease, lung disease, high blood pressure, renal failure, advanced diabetes or other diseases where small changes in condition have large health risks.
