Piedmont Home Care is a not-for-profit organization caring for
patients in their own homes. Under a physician's plan of care, skilled
health care professionals make home visits that may prevent hospitalization
or long term care placement.

Who is Eligible for Home
Care?
You may be if you meet one or more of these criteria:
- You are between the ages of newborn and older adult.
- You are in need of skilled care that only a healthcare professional
can provide.
- You are under a doctor's care.
- You have Medicare or Medicaid and are homebound.
- You only leave home for short, infrequent trips to receive medical
care and it takes a lot of effort for you to leave your home.
What Can Home Care Provide?
- Have nurses available by phone 24 hours a day, 7 days a week for
questions and home visits while the patient is recovering from an
illness or surgery.
- Provide teaching for the patient and caregiver.
- Provide physicians with information about how patients are doing
when they are unable to visit their offices.
- Provide information on other community resources and assist with
making referrals to them.
- Assist with personal care needs such as bathing, grooming, and dressing.
- Assist with ways to make the home more manageable when the patient
has limited mobility.
